In order to work in a culinary, security, or in a child care position (just to name a few) you will
be required to obtain a Health Card.

Please use the following procedure to obtain a health card:
- Come to the Main Health Center at 625 Shadow Lane or the East Las Vegas Public Health Center at 560
N. Nellis, Suite E-12 between the hours of 8:00 a.m. and 4:00 p.m.. Monday through Friday. There will be a fee of
$30.00.
- If you want your social security number on the card, you must provide valid proof of social security identification, such as your
social security card or a driver's license that shows your social security number.
- Once you arrive, you will fill out an application form. Or you can bring a form that you have already printed and filled out.
- If you are getting a Food Handler Card, you will receive a Hepatitis A vaccination.
- If you are getting a Child Care Card or an Adult Group Care Card, you will receive a Hepatitis A vaccination as well as either a skin test
or x-ray for Tuberculosis.
- You must also attend an hour and a half class along with a movie within 30 days of applying for the Health Card.
- Check the schedule for the time most convenient for you. Reservations are not necessary.
- You will be given a temporary card to use until you complete the class.
- Your Health Card must be renewed every three years.
- To renew the card, visit one of the two Health Centers. If you renew your card before it expires, bring it with you and there will be a fee of $30.00.
If you renew your card after it has already expired, bring it with you and there will be a fee of $35.00.
*Duplicate Health Cards are available to those who have lost their Health Card, for a fee of $10.00.
**Local checks are accepted with check card and photo identification.
In order to work on the premises where liquor and gaming are present, you must have a Sheriff's Work Card.
You cannot obtain the card until you are already employed, upon which you will be referred by your new employer.
Requirements:
- You must be at least 21 years of age if employed in a gaming position or dispense alcohol.
- If under the age of 25 years, you must show your birth certificate.
- If you are not a citizen of the United States, you must present proper documentation from Immigration and Naturalization Services.
- If under the age of 18 years, you must have a juvenile work permit .
- You cannot have an extensive criminal history, outstanding arrest warrant, or have had a work card previously revoked by the Gaming Control
Board.

Anyone who serves or sells alcohol or works in security must have a
TAM (Techniques of Alcohol Management) Card
